Required Forms for Mt Carmel Child Development Center
Please note ALL forms are required for new and returning students prior to your child's first day of attendance.

                  

Health Inventory Form and Immunization Form (requires physician to fill out a portion)

You will be required to update your child's medical forms as needed.

Lead Testing

Emergency Contact Form

All About Me Form, Information about your child's likes, dislikes, strengths, etc.

Medication Administration Form - Required for use of any medications or over the counter products, including sunscreen (while in the care of the center).

Please send in the registration form with the required payment. All other forms may be sent in electronically before the start of class. Non-Refundable Registration/ Activity Fee of $125.00 is required with the first month's tuition for all students.
